Indications/Uses
Herpes simplex virus infections of the skin & mucous membrane including initial & recurrent genital herpes; varicella infections (chicken pox); herpes zoster (shingles). Prevention of herpes simplex virus infections in immunocompromised patients including recurrent infections. Management of severely immunocompromised patients including those w/ advanced HIV disease (CD4+ counts <200/mm3, including patients w/ AIDS or severe AIDS-related complex) or following bone marrow transplantation.
Dosage/Direction for Use
Adult Acute treatment of herpes 800 mg every 4 hr, 5 times daily (omit last night time dose) for 7-10 days. Treatment of herpes simplex infections including genital herpes 200 mg every 4 hr, 5 times daily (omit last night time dose) for 7-10 days. Immunocompromised patient (eg, after bone marrow transplant) May be increased to 400 mg every 4 hr, 5 times daily (omit last night time dose). Suppression of herpes simplex infections including chronic suppressive therapy for recurrent disease 200 mg every 4-6 hr OR 400 mg every 12 hr. May titrate down to 200 mg every 8-12 hr. Treatment cycle: 6-12 mth. Prophylaxis of herpes simplex infections Immunocompromised patient (eg after bone marrow transplant) 200 mg every 6 hr. May be increased to 400 mg if needed. Treatment of varicella & herpes zoster 800 mg every 4 hr, 5 times daily (omit last night time dose) for 7 days. Severely immunocompromised patient 800 mg every 6 hr for 6-12 mth. Treatment of varicella infections Childn >6 yr 800 mg every 6 hr for 5 days, 2-6 yr 400 mg every 6 hr for 5 days. Treatment & prophylaxis of herpes infections Immunocompromised childn ≥2 yr 200-400 mg every 4 hr, 5 times daily (omit last night time dose), <2 yr 100-200 mg every 4 hr, 5 times daily (omit last night time dose).
Administration
May be taken with or without food.
Special Precautions
Renal impairment. Concomitant use w/ other potentially nephrotoxic drugs. Not used to cure herpes & prevent transmission of infection. Pregnancy & lactation. Childn <2 yr. Elderly.
Adverse Reactions
Increased BUN &/or serum creatinine conc, anuria, hematuria; headache, aggressive behavior, agitation, ataxia, coma, confusion, decreased consciousness, delirium, dizziness, encephalopathy, hallucinations, obtundation, paresthesia, psychosis, seizures, somnolence, tremors; nausea, vomiting, diarrhea; anemia, leukocytoclastic vasculitis, leukopenia, lymphadenopathy, thrombocytopenia, fever, pain.
Drug Interactions
Synergistic effect w/ ketoconazole & interferon. Increased t½ & AUC & decreased renal clearance w/ probenecid. Neurotoxic effects w/ zidovudine.
